What are mental health supports?

 

A Medicaid funded service that:

 

●Provides training in or reinforcement of functional skills and appropriate behavior related to health, safety, activities of daily living and access to community resources;

●Supports medication management and related issues;

●Monitors health both mental and physical;

●Monitors nutrition and physical condition;

 

It is an adjunct service to psychiatric and other service providers who care for the individual.

 

Who qualifies for mental health supports?

 

Anyone who receives Medicaid, who has a mental health issue and meets specific risk criteria, has an existing Axis I diagnosis or who qualifies for the same, and who is at least sixteen years of age or above.

 

What is the referral process?

 

Anyone can make the referral for services, including the individual, who may self-refer.

 

We accept referrals for individuals living in the following cities and counties:  NRVSCB region including the city of Radford, counties of Floyd, Giles, Montgomery, and Pulaski, and the BRBH region including the Cities of Roanoke and Salem, and counties of Botetourt, Craig, and Roanoke.
